RN Patient Care Manager

We are seeking a highly skilled and compassionate RN Patient Care Manager to join our team at Ochsner Home Health of Raceland. As a Patient Care Manager, you will play a vital role in ensuring the delivery of high-quality patient care and services in the comfort of patients' homes.
Key Responsibilities- Supervise and Coordinate Clinical Services: Oversee the provision of nursing care to patients in their homes, ensuring that services are delivered in accordance with physician orders and applicable laws and regulations.
- Provide Clinical Leadership: Direct and coordinate the work of clinicians, ensuring that patient needs are met and care is individualized to each patient's requirements.
- Coordinate Patient Care: Receive referrals, ensure appropriate clinician assignments, evaluate patient orders, and plot start of care visits.
- Ensure Patient Needs are Met: Continuously assess patient needs and ensure that care rendered is individualized, appropriate, and reasonable, meeting home health eligibility criteria and physician orders.
- Review and Improve Care: Review assessments and plans of care daily, per assigned workflow, and consult with clinicians with recommendations as needed.
- Current RN Licensure in LA
- Current CPR Certification
- Current Driver's License, Vehicle Insurance, and Access to a Dependable Vehicle or Public Transportation
