About the Role

This is an opportunity to join a dynamic team of healthcare professionals who are dedicated to making a difference in the lives of others. The ideal candidate will possess a Master's Degree in Social Work from a CSWE-accredited institution and have at least one year of experience as a Social Worker in a healthcare or hospice setting.

The successful candidate will be able to demonstrate a strong understanding of the psychosocial dynamics of illness, loss, and death, as well as the ability to apply knowledge of the special needs of hospice patients and their families. They will also possess excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and other healthcare professionals.

Key Responsibilities
  • To provide comprehensive psychosocial assessments and develop individualized plans of care that address the unique needs and goals of each patient and family.
  • To coordinate and implement interventions that promote the physical, emotional, and spiritual well-being of patients and their families.
  • To communicate effectively with patients, families, and other healthcare professionals to ensure seamless coordination of care.
What We Offer

As a valued member of our team, you can expect a competitive salary range of $59,885 to $74,860, depending on location, experience, education, and qualifications.
