In this role, you will play a critical part in ensuring the accuracy and completeness of patient medication histories, working closely with pharmacists and other healthcare professionals to provide exceptional patient care.
Key Responsibilities- Medication History Collection: Identify and prioritize patients requiring medication history collection, review and collect necessary information, and interview patients to gather comprehensive medication histories.
- Documentation and Tracking: Accurately document medication histories and allergies in the electronic health record, track collection progress, and notify relevant staff of completion.
- Infection Control and Safety: Adhere to established procedures for infection control, safe patient practices, interpreter services, and contraindications to patient interviews.
- Quality Control and Maintenance: Maintain a safe and clean working environment, perform quality control activities, and ensure compliance with policies, laws, and regulations.
- Assistance and Support: Provide support to the Acute Pharmacy Technician as needed, including preparing, charging, and crediting medications, restocking automated dispensing technology, and compounding specialty medications and intravenous medications.
- Pharmacy Technician Licensure and Certification: Possess active Pharmacy Technician certification or licensure in the state of practice.
- Experience and Knowledge: Minimum of two years pharmacy technician experience, knowledge of brand/generic names of drugs, OTCs, and herbal/dietary supplements, and ability to complete mathematical calculations.
- Communication and Interpersonal Skills: Strong customer service focus, exceptional communication skills, and ability to interface with all levels of staff, physicians, patients, and other contacts.
- Community Pharmacy Experience: Previous experience in community pharmacy settings.
- Additional Education and Experience: Additional related education and/or experience preferred.
