Resumen del Puesto
El Coordinador de Programa CalAIM es responsable de la implementación general y las operaciones continuas de los servicios CalAIM, incluyendo la Gestión de Casos Mejorada y los Soportes Comunitarios, tales como Servicios de Tenencia de Vivienda y Servicios de Navegación de Vivienda. Este rol implica la gestión directa de personal no asignado y la coordinación de la gestión del personal que proporciona servicios CalAIM integrados en otros programas. El Coordinador es responsable de la gestión de productividad, el cumplimiento del presupuesto y la conformidad general con los estándares de CalAIM.
Funciones y Responsabilidades Esenciales
Las funciones esenciales incluyen, pero no se limitan a lo siguiente:
- Actuar como el Director del Programa ECM según las pautas de ECM.
- Monitorear y gestionar los presupuestos del programa de manera adecuada.
- Gestionar la productividad del personal para asegurar que se logren los estándares mínimos de facturación mensualmente.
- Capacitar y apoyar al personal en la documentación y entrega de servicios, incluyendo requisitos específicos de CalAIM, Cuidado Informado por Trauma, Vivienda Primero y Reducción de Daños.
- Presentar sobre los programas en la comunidad según se solicite.
- Ejecutar un alto nivel de comunicación proactiva con los Gerentes de Programa de empleados integrados.
- Coordinar revisiones de casos de todos los clientes de CalAIM, al menos una vez al mes, con mayor frecuencia según sea necesario.
- Asegurar que la facturación mensual se envíe de manera oportuna y precisa.
- Asegurar que los informes mensuales, incluyendo informes de personal y otros informes solicitados, se envíen de manera oportuna y precisa.
- Reclutar, entrevistar y recomendar candidatos para contratación o en colaboración con los Gerentes de Programa para posiciones integradas.
