Key Responsibilities- Assess and reassess patients' needs, developing and implementing individualized care plans to promote optimal rehabilitation outcomes.
- Administer medications and treatments as prescribed by physicians, ensuring accurate and timely delivery.
- Collaborate with interdisciplinary teams to coordinate patient care, including therapy sessions, medical procedures, and discharge planning.
- Provide education and support to patients and their families, promoting independence and self-care skills.
- Participate in quality improvement initiatives, contributing to the hospital's commitment to excellence in patient care.
- Graduate of an accredited Licensed Practical/Vocational Nurse program.
- Current state licensure as a Licensed Practical/Vocational Nurse.
- CPR/BCLS certification, with ACLS preferred.
- IV Certified or obtained within 6 months, highly preferred.
- Minimum six months' Medical/Surgical experience in an acute care setting, preferred.
