Job Summary

We are seeking a skilled Community Health Navigator to join our team at Providence California Regional Services. This role is responsible for helping community residents and Medical Center patients and their families navigate and access community services, other resources, and adopt healthy behaviors.

Responsibilities
  • Collaborate with community residents, patients, and their families to identify needs and develop plans to access community services and resources.
  • Provide education and support to promote healthy behaviors and improve health outcomes.
  • Work closely with healthcare providers to ensure seamless transitions and continuum of care.
Requirements
  • Bachelor's degree in a related field or equivalent experience.
  • 1 year of experience in a combination Community Health Worker I and II position, or 2 years related community outreach, enrollment, education or other community health administrative experience.
  • Valid driver's license and proof of auto insurance required.
