Home Health LPN Liaison Opportunity

ChristianaCare HomeHealth is seeking a dedicated and compassionate LPN Liaison to join our team in Southern Delaware. As a key member of our Home Health team, you will play a vital role in ensuring a smooth transition for patients from acute care and post-acute care settings.

Key Responsibilities:
  • Collaborate with discharge planners, case managers, and patients to identify home care needs and develop personalized care plans.
  • Provide education and support to patients and families on home health services, including virtual visits and adult day programs.
  • Communicate effectively with referral sources, physicians, and other healthcare professionals to ensure seamless care transitions.
  • Coordinate with internal departments to schedule start-of-care visits and ensure timely patient assessments.
  • Serve as a subject matter expert on HomeHealth services, providing guidance and support to referral sources and patients.
Requirements:
  • LPN licensure in the State of Delaware or an equivalent state.
  • 1 year of experience in home health, hospice, or care coordination preferred.
  • Knowledge of ChristianaCare HomeHealth service lines and policies.
