Job Title: Home Health Occupational Therapist

LHC Group is seeking a skilled Home Health Occupational Therapist to join our team in Glen Ridge, New Jersey.

About the Role:

As a Home Health Occupational Therapist, you will be responsible for assessing and evaluating patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment. Based on this assessment, you will help determine a treatment plan, perform interventions aimed at improving and enhancing the patient's well-being, and evaluate their progress.

Key Responsibilities:
  • Evaluate patient's functional status, status of all body systems as required for CMS documentation, and occupational therapy needs.
  • Consult with the physician in the development of the therapy plan of care.
  • Instruct and inform participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress towards goals.
  • Complete progress notes for each visit and submit them per LHC policy.
  • Report outcomes of evaluation, goals, and anticipated projected frequency of care to nurse supervisor within 24 hours of the evaluation.
  • Participate in staff conferences and committees as requested by the agency.
Requirements:
  • Current Occupational Therapy licensure in the state of New Jersey.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
