Job Title: Occupational Therapist - Home Health

LHC Group is seeking a skilled Occupational Therapist to join our team in Glen Ridge, New Jersey.

About the Role:

We are looking for a dedicated and compassionate Occupational Therapist to provide high-quality care to our patients in the comfort of their own homes. As an Occupational Therapist with LHC Group, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities:
  • Evaluate patients' functional status and occupational therapy needs
  • Develop and implement individualized treatment plans to improve patients' functional abilities and quality of life
  • Provide occupational therapy interventions, including adaptive equipment training and home assessments
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Document patient progress and outcomes
Requirements:
  • Current Occupational Therapy licensure in the state of New Jersey
  • Current CPR certification
  • Current driver's license, vehicle insurance, and access to a dependable vehicle
  • At least 1 year of experience as an Occupational Therapist
