Key Responsibilities:
Regular outreach to discharge coordinators to provide education and enhance awareness of our services
Identify opportunities to establish ourselves as the preferred provider through relationship development and constructive feedback
Collaborate with internal field colleagues to streamline the admission process and implement best practices
Facilitate necessary communication between hospitals, admissions, and clinic teams to navigate patient placements with unique circumstances
Ensure a smooth and seamless discharge experience for patients and hospital personnel to DaVita centers
Develop an in-depth understanding of the local market

Minimum Qualifications:
A Bachelor's Degree is preferred
Willingness to travel to various locations to engage with partners (both internal and external) as needed
Possession of a valid driver's license in the state of residence and an insurable driving record
Intermediate proficiency in various software applications, including but not limited to Microsoft Word, Excel, PowerPoint, and billing systems
