We are seeking a highly skilled Physical Therapist to join our team at Creative Solutions in Healthcare. As a Physical Therapist, you will play a vital role in providing person-centered care to our residents, helping them achieve positive outcomes and improve their quality of life.

Responsibilities:

  • Consult with patients to learn about their physical condition and develop personalized rehabilitation programs.
  • Assess and interpret patient evaluations and test results to create effective treatment plans.
  • Deliver medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
  • Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
  • Document patient progress and ensure compliance with company and state/federal regulatory requirements.

Requirements:

  • Active/Valid Texas license in Physical Therapy.
  • SNF/Long term care experience preferred.
  • Current knowledge of treatment practices.
  • Ability to manage patients with different types of personalities.
