Licensed Clinical Social Worker

Sun River Health is seeking a full-time Licensed Clinical Social Worker to join our team at the A31 Sidney Baer (NYC) location.
Job Summary:The Licensed Clinical Social Worker will coordinate and oversee all aspects of psychosocial programming, including support groups for caregivers and participants. The ideal candidate will have a Master's degree in Social Work and at least 4 years of related work experience. The salary range is $65,000 - $100,000 annually, depending on licensure and experience.
Responsibilities:- Assist in planning, developing, and evaluating the psychosocial needs and concrete needs of program clients.
- Coordinate and oversee all aspects of psychosocial programming and services.
- Ensure that admission and discharge procedures are consistent with individual needs.
- Ensure completion of psychosocial assessments of clients within the mandated time frame.
- Participate in the interdisciplinary team care planning process.
- Carry a reduced case management caseload, as necessary.
- Serve as a resource for all other program staff; develop and maintain resource file.
- Lead or co-lead psychosocial groups, including support groups for family members, significant others, and caregivers.
- Provide psychotherapy for a select caseload.
- Perform interagency liaison work with other service agencies, as needed.
- Maintain client case notes and statistical records; ensure documentation of progress notes.
- Participate in Quality Assurance program to evaluate, monitor, identify problems, and institute revisions to maintain quality of client care and provide services in accordance with current standards of professional practice.
- Attend and participate in seminars and continuing education programs to expand knowledge of current trends in the field of specialty and in issues concerns of the HIV population.
- Attend all mandatory in-service programs.
- Master's degree in Social Work.
- Minimum Related Work Experience: 4 years.
- NYS Licensed LMSW, LCSW, or LMHC.
