We're committed to providing whole-person care, facilitating timely follow-up, and care coordination to ensure the best possible outcomes for our patients.
What You'll Do:- Work collaboratively with our clinical care team to deliver in-home care services, utilizing your expertise in acute care to provide comprehensive health assessments and treatment plans.
- Perform a range of clinical tasks, including splinting, suturing, wound care, and minor procedures, as needed.
- Utilize critical thinking skills and evidence-based standards of practice to diagnose and treat complex illnesses.
- Accurately document patient encounters and ensure accurate and thorough documentation.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Minimum of two years of experience as an APP in acute care settings, such as ER, Urgent Care, or Internal Medicine.
- Graduate of an accredited institution with an advanced degree.
- Current, unrestricted state license as a Nurse Practitioner or Physician Associate.
- Nationally board certified, current.
- Current BLS required, ACLS certification preferred.
- Prescriptive authority and DEA.
- Valid driver's license with clean driving record.
