- Bathe and clean residents as needed, using a gentle and respectful approach to maintain their dignity.
- Assist with personal care tasks, such as grooming, dressing, and feeding, while promoting independence and autonomy.
- Monitor and report any changes in a resident's physical or mental condition to the charge nurse, ensuring prompt attention to their needs.
- Document care provided in the resident's medical record, maintaining accurate and detailed records of their care.
- Collaborate with the healthcare team to develop and implement individualized care plans that meet the unique needs of each resident.
- Provide emotional support and companionship to residents, engaging in activities that promote their physical and mental well-being.
- Maintain a clean and safe environment, adhering to infection control protocols and ensuring the proper use of personal protective equipment.
- Participate in ongoing education and training to enhance your skills and knowledge, staying up-to-date on the latest best practices in home health care.
Requirements:
- Current certification as a Home Health Aide or Certified Nursing Assistant.
- Experience working in a home health care setting, preferably with seniors or individuals with disabilities.
- Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals.
- Ability to lift, transfer, and position residents with care and precision, using proper body mechanics and equipment.
- Flexibility to work a variety of shifts, including evenings, weekends, and holidays, as needed.
