About the Role:We are looking for a dedicated and compassionate Case Manager RN to provide high-quality care to our patients. As a Case Manager RN, you will be responsible for assessing the coordination and access to needed medical, psychiatric, social, educational, and other services essential for meeting the health needs of the individual.
Key Responsibilities:- Assess and coordinate assessment, treatment planning, and return to work processes with the employee, providers, and employer.
- Coordinate and provide care that is safe, timely, effective, efficient, equitable, and patient-centered.
- Maintain collaborative working relationships with other community providers to develop specialized services as required.
- Promote quality and cost-effective interventions and outcomes.
- Provide consultation to staff on specific cases.
- Prepare and maintain written case records, reports, and forms, perform case follow-up and closing, and perform other administrative tasks as required.
- Certificate from an accredited Nursing School or Associate Degree (A.S.) Bachelor's Degree (BSN) from four-year college or university preferred.
- Current hands on training in AHA or ARC Basic Life Support for health care providers is required. Advanced Cardiac Life Support may also be required based on contract scope of services.
- Case Management (CCM) Certification, AANC CM, American Academy of Case Management (AACM), COHN-S/CM, Certification in Occupational Health Nursing, Certification in Emergency Nursing, or Certificate in Case Management preferred.
- At least three years experience as a Registered Nurse in an emergency room, occupational health clinic or other ambulatory care clinic.
- Occupational health experience required.
