Job Summary:

Liberty Health is seeking an experienced Hospice Care Specialist to join our team. As a Palliative Nurse Practitioner, you will play a critical role in providing state-of-the-art care and pain management services to our patients and families facing terminal illness.

Key Responsibilities:

  • Implement palliative care program requirements, ensuring seamless coordination with our interdisciplinary team.
  • Conduct face-to-face encounters with patients to provide re-certification information to our Medical Director/Associate Medical Director.
  • Communicate clinical findings to primary care physicians and supervising Medical Director/Associate Medical Director.
  • Collaborate with patients, families, and agency personnel to ensure comprehensive care.

Requirements:

  • Master's degree in Nursing, with current licensure in the state of practice.
  • Certification as an Adult or Family Nurse Practitioner.
  • Experience in Hospice or Palliative Care nursing is preferred.
  • Knowledge of the framework, function, and organization of a hospice care program.
  • Excellent communication and organization skills.
