Job Summary:

We are seeking a compassionate and skilled LPN/LVN to join our team at Conviva Legacy Place. The ideal candidate will have a strong background in patient assessment, medication administration, and nursing care planning. Responsibilities:
  • Assess patient health problems and needs
  • Takes vital signs, administers medication, develops, and implements nursing care plans, and maintains medical records
  • May advise patients on health maintenance and disease prevention or provide case management
  • Understands own work area professional concepts/standards, regulations, strategies, and operating standards
Requirements:
  • Current state licensure as an LPN or LVN
  • 1-2 years' experience as LPN or LVN in an inpatient or outpatient clinic setting
  • IV Certified
  • Experience in a fast-paced/high-volume environment
  • Must be able to work at the Conviva Care clinic located at: 18414 US Hwy 281 N, Suite 104 San Antonio, TX 78259
Preferred Qualifications:
  • Hands-on professional Phlebotomy experience
  • Bilingual in English and Spanish, with the ability to speak to patients in both languages
  • Experience with Electronic Medical Records
  • Experience with HEDIS
  • Knowledge of primary care medicine requirements (state specific)
  • Working knowledge of clinical operations
