Responsibilities:- Assess patient health problems and needs
- Takes vital signs, administers medication, develops, and implements nursing care plans, and maintains medical records
- May advise patients on health maintenance and disease prevention or provide case management
- Understands own work area professional concepts/standards, regulations, strategies, and operating standards
- Current state licensure as an LPN or LVN
- 1-2 years' experience as LPN or LVN in an inpatient or outpatient clinic setting
- IV Certified
- Experience in a fast-paced/high-volume environment
- Must be able to work at the Conviva Care clinic located at: 18414 US Hwy 281 N, Suite 104 San Antonio, TX 78259
- Hands-on professional Phlebotomy experience
- Bilingual in English and Spanish, with the ability to speak to patients in both languages
- Experience with Electronic Medical Records
- Experience with HEDIS
- Knowledge of primary care medicine requirements (state specific)
- Working knowledge of clinical operations
