About the Role:

Conviva is seeking a dedicated and compassionate LPN/LVN to join our team at Legacy Place. As a Licensed Vocational Nurse/Licensed Practice Nurse, you will play a vital role in providing high-quality patient care and making a positive impact in the lives of our senior members.

Key Responsibilities:

  • Assess patient health problems and needs
  • Takes vital signs, administers medication, develops, and implements nursing care plans, and maintains medical records
  • May advise patients on health maintenance and disease prevention or provide case management
  • Understands own work area professional concepts/standards, regulations, strategies, and operating standards

Requirements:

  • Current state licensure as an LPN or LVN
  • 1-2 years' experience as LPN or LVN in an inpatient or outpatient clinic setting
  • IV Certified
  • Experience in a fast-paced/high volume environment

Preferred Qualifications:

  • Hands-on professional Phlebotomy experience
  • Bilingual in English and Spanish, with the ability to speak to patients in both languages
  • Experience with Electronic Medical Records
  • Experience with HEDIS
  • Knowledge of primary care medicine requirements (state specific)

Working Hours:

Monday to Friday 8AM-5PM
