Job Summary

We are seeking a highly motivated and organized Registered Nurse to join our team as a Home Hemodialysis Registered Nurse (HHD RN). As a key member of our healthcare team, you will be responsible for providing comprehensive patient care in an independent setting, empowering patients to take control of their health, and leading, coaching, and guiding with your nursing skills.

Key Responsibilities
  • Provide education on Home Hemodialysis to patients and their care partners, enabling them to dialyze from home.
  • Manage patient cases independently, ensuring their treatment stays on track and providing ongoing care.
  • Lead and coach patients, support systems, and care partners, building deep relationships and fostering a sense of community.
  • Work independently, with flexibility to manage your own schedule and cases.

Requirements
  • Current Registered Nurse (RN) license in the state of practice.
  • Minimum of 12 months of RN experience.
  • Current driver's license, reliable transportation, and vehicle insurance in the state of residence.
  • Associates Degree in Nursing (A.D.N) from an accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from an accredited diploma program may be substituted for nursing degree.
  • Basic computer skills and proficiency in MS Word and Outlook.
Preferred Qualifications
  • Previous Chronic Hemodialysis, Acute Dialysis, Peritoneal Dialysis (PD) or Home Hemodialysis (HHD) nursing experience.
  • Training/Teaching/Healthcare Education background.
  • Previous case management experience.
  • Familiarity with, or excitement to learn about telehealth technologies.
  • Certified Nephrology Nurse (CNN), Certified Dialysis Nurse (CDN).
