Job Summary

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our integrated Care Team. As a key member of our team, you will play a vital role in delivering personalized, compassionate medical care to patients in the comfort of their own homes. Your expertise will be instrumental in helping patients improve their quality of life, slow the progression of kidney disease, and achieve positive health outcomes.

Responsibilities
  • Conduct comprehensive annual wellness exams on patients in their homes and virtually
  • Counsel and educate patients and families about benefits and programs available to help them live healthier lives
  • Document patient information, including chief complaint, diagnosis, medical history, and treatment plans
  • Coordinate care with primary care providers, specialists, and ancillary services
  • Prescribe medications, order tests, and collaborate with patients' Monogram physicians
  • Deliver evidence-based, timely care that reduces avoidable hospitalizations and maximizes quality of life
  • Participate in integrated care team meetings and contribute to the development of treatment plans
Requirements
  • Bilingual (English/Spanish) highly preferred
  • Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
  • Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
  • Current and unrestricted DEA certificate
  • Ability to work autonomously and practice without direct supervision
  • Access to transportation, a valid driver's license, and car insurance
  • Proficiency with medical instruments and equipment required by the work
  • Knowledge of computer-based data management programs and information systems
  • Ability to communicate effectively with patients, families, physicians, and community representatives
  • Sound understanding of federal and state regulations, including HIPAA and OSHA
  • 2 or more years of direct patient care experience
  • Managed Care/IPA/Health Plan experience
