As a Clinical Supervisor at Interim HealthCare, you will oversee a team of home health professionals, ensuring the delivery of high-quality patient care. With a focus on compliance and patient satisfaction, you will be responsible for managing a caseload of patients, supervising staff, and making clinical decisions to optimize patient outcomes.

Key Responsibilities:

  • Supervise and mentor home health staff to ensure adherence to company policies and procedures.
  • Conduct regular patient assessments to identify areas for improvement and develop individualized care plans.
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
  • Manage and maintain accurate records and reports, including patient charts and staff performance evaluations.

Requirements:

  • Graduate of an accredited Physical Therapist program and active PT license in Colorado.
  • Minimum of three years of physical therapy experience in home healthcare, with a focus on leadership and management.
  • Two years of management experience, with a proven track record of success in supervising staff and improving patient outcomes.
  • CPR certification and knowledge of state and federal home health regulations.
