About the Role

The Care Coordinator will be essential in coordinating caregiver services for our clients, reporting to the Director of Client Services to ensure each client is matched with the right caregivers for their needs. This is an in-office, salaried position requiring reliable transportation for occasional client-related deliveries.

Main Responsibilities
  • Caregiver Coordination: Support the scheduling and assignment of caregivers, ensuring all shifts are appropriately covered, and caregivers are well-matched to clients' needs.
  • Client Care Matching: Collaborate with the Director of Client Services to implement client care plans effectively, ensuring that the right caregivers are assigned for optimal client satisfaction.
  • Recruitment and Hiring: Coordinate with the recruitment team to hire caregivers and conduct final interviews to onboard qualified caregivers.
  • Document Management: Ensure that all client care and caregiver records are meticulously documented in compliance with company policies and state regulations.
  • Client Deliveries: Occasionally handle the delivery of essential items to clients' homes, ensuring timely and accurate service.
  • Strategic Staffing: Work with the recruitment team to ensure an adequate distribution of caregivers across designated zones or locations of interest to the agency, optimizing care coverage in each area.
Requirements and Qualifications
  • Experience: Minimum of 1 year in homecare and 1 year of office experience. Experience in caregiver coordination or a similar role is preferred.
  • Skills: Strong organizational abilities, excellent communication skills, and proficiency with app-based management tools. Interviewing experience is a plus.
  • Certification: A valid Certified Home Health Aide (HHA) certification is a plus.
  • Attributes: Highly organized, proactive, flexible, and able to work independently while collaborating effectively within a team.
  • Requirements: Reliable transportation is required for occasional deliveries. Proven ability to work with a diverse background of caregivers and personalities, adaptable to fostering encouragement and guidance.
Compensation and Benefits
  • Estimated Salary: $62,000 - $72,000 per year, depending on experience.
