Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at Foundation Health Partners. As a key member of our healthcare team, you will provide high-quality patient care under the supervision of a registered nurse and/or licensed physician.

Responsibilities
  • Conduct thorough patient assessments, identifying physical, psychological, social, and educational needs.
  • Develop and implement individualized care plans, collaborating with the multidisciplinary team to ensure comprehensive care.
  • Provide direct patient care, including administering medications, performing treatments, and educating patients and families on health management.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Document patient progress, including observations, interventions, and outcomes, in accordance with regulatory requirements and facility policies.
Requirements

To be successful in this role, you must possess a current, valid LPN or LVN license in the state of practice, as well as BLS certification. Additional certifications or continuing education may be required based on area of practice.
