Nurse Practitioner

We are seeking an experienced and skilled Nurse Practitioner to join our team at Regency Integrated Health Services. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and services to our residents and patients.
Key Responsibilities- Provide Provider-level care coordination and management for assigned patients, ensuring timely and effective delivery of healthcare services.
- Develop and implement individualized care plans, taking into account the unique needs and goals of each patient.
- Conduct thorough assessments, diagnose conditions, and develop treatment plans in collaboration with attending physicians and other healthcare professionals.
- Prescribe medications and treatments as authorized by state law and within the scope of your practice.
- Collaborate with facility staff, families, and other healthcare providers to ensure seamless care coordination and communication.
- Participate in on-call rotations to ensure continuity of care and provide support to patients and families outside of regular business hours.
- Stay up-to-date with the latest medical research, technologies, and best practices, applying this knowledge to improve patient outcomes and quality of care.
- Current licensure as a Registered Nurse (RN) and Advanced Practice Registered Nurse (APRN) in the state of Texas.
- Board Certification in a relevant specialty.
- Experience working in a long-term care or rehabilitation setting, with a focus on gerontology or geriatrics.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to maintain accurate and detailed records, including patient charts and medical histories.
