About the Role: As a member of our Home and Community Care team, you will provide annual clinical assessments to patients in their homes, identifying health risks and coordinating follow-up care. This is a preventive care role that makes a significant impact on patient well-being.

Responsibilities:
  • Conduct comprehensive home visits, including medical history evaluations, system reviews, medication reconciliation, vital signs, and physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as needed.
  • Identify diagnoses for care management and active medical management.
  • Communicate findings to inform members' primary care physicians about potential care gaps.
  • Address urgent and emergent situations appropriately.
  • Provide education on disease processes, medications, and compliance.

Requirements:
  • Active NP license or ability to obtain by start date; licensure must be unencumbered.
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care, or ability to obtain certification by start date.
  • Ability to complete physical job requirements, including lifting a 30-pound bag and navigating various dwelling conditions.
  • Reliable transportation for home visits.
