Your Responsibilities:
  • Conduct comprehensive home visits, incorporating thorough medical histories, system reviews, medication reconciliations, vital signs, and physical examinations.
  • Implement evidence-based practice screenings, including point-of-care testing where necessary.
  • Formulate lists of current and past medical conditions using clinical judgment and assessment findings.
  • Communicate assessment results to inform members' PCPs about potential care gaps.
  • Respond to urgent and emergent situations effectively.
  • Provide educational resources on disease processes, medications, and adherence.

Requirements and Qualifications:
  • Hold an active NP license or be able to obtain one by your start date, with a clear record.
  • Possess active ANCC or AANP national certification in your area of expertise or be able to obtain it by your start date.
  • Meet the physical demands of the job, including heavy lifting and navigating various living conditions.
  • Have reliable transportation for home visits.
