Job SummaryWe are seeking a skilled and compassionate Palliative Care RN to join our team. As a key member of our interdisciplinary team, you will work with patients, their families, and other healthcare professionals to provide and coordinate high-quality end-of-life care. Your primary role will involve conducting in-person and telephonic assessments, providing education to patients and families, and ensuring patients are comfortable in their home environment.
Responsibilities- Conduct in-person and telephonic RN assessments and screenings to identify patient support needs
- Provide education to patients and families on disease process, self-care techniques, end-of-life care, medications, nutrition, and dietary needs
- Communicate findings to the PCP or AIM Palliative NP and coordinate care with palliative team members
- Support hospice referral as appropriate and document timely clinical records on all patients
- Develop and implement individualized care plans for palliative care patients
- Collaborate with the interdisciplinary team to provide comprehensive care and support
- Nursing experience in palliative care, oncology, hospice, or geriatric care preferred
- Additional experience in hospital settings, such as Staff RN, Med Surg, PACU, Float pool, ER, or critical care, is also desirable
- Fluent competence in computer programs Microsoft Outlook, Microsoft Word, and Microsoft Excel
- Fluent in electronic medical record documentation
