We are seeking a highly skilled LPN/RN to join our team at AltaPointe Health as an Outpatient Family Medicine Nurse. The successful candidate will be responsible for providing high-quality patient care, working collaboratively with our multidisciplinary team, and contributing to the development of our interdisciplinary treatment plans.
ResponsibilitiesClinical Responsibilities:
- Assist with the completion of patient assessments, including medical checklists, physical health screenings, and medication reconciliation.
- Administer medications, ensuring adherence to the Seven Rights of medication administration.
- Provide patient education on disease processes and medication use.
- Monitor patients for potential medication side effects and report findings to physicians.
- Provide direct patient care, evaluate outcomes, and consult with specialists as needed.
- Accurately assign triage categories and maintain accurate nursing documentation.
- Identify and address psychosocial, cultural, ethnic, and religious/spiritual needs of patients and families.
- Communicate effectively with physicians regarding patient physical and mental conditions.
- Administer medical and psychiatric screenings as needed.
- Complete medical checklists, vital signs, and other tasks as indicated.
- Perform ancillary testing and tasks as ordered by physicians.
- Give injections or treatments and perform routine laboratory tests.
- Supervise nursing functions and ensure conformance with established standards and procedural requirements.
- Assist patients with assistance programs and referrals, and maintain updated files.
- Respond to consumer requests for medication refills.
- Assist with meeting reporting requirements of SAMHSA grants.
- Review charts for treatment compliance and follow up on missed injections.
- Interface with the multidisciplinary team and participate in the development of interdisciplinary treatment plans.
- Monitor laboratory results and report abnormal findings to physicians.
- Provide emergency care as needed, according to Accordia Health Policy and Procedures.
- Demonstrate crisis intervention skills and de-escalation techniques.
- Participate in general and medical staff meetings as required.
- Key data into computer systems to maintain office and patient records.
- Educate the community and patients on health issues to increase health literacy.
- Conduct infection control for staff and consumers as needed.
- Perform related duties as assigned.
Supervision and Consultation:
- Seek clinical supervision and consultation as needed.
- Accept and employ suggestions for improvement.
- Actively work to enhance clinical skills.
Clinical Record Keeping:
- Document in a timely fashion, according to Accordia Health policy.
- Document clearly and concisely, including patient problems, treatment goals, and services provided.
- Ensure documentation is accurate, up-to-date, and completed promptly.
- Document legibly.
Courteous and Respectful Attitudes:
- Treat patients with care, dignity, and compassion.
- Respect patient privacy and confidentiality.
- Be pleasant and cooperative with others.
- Assist patients and visitors as needed.
- Be sensitive to patient needs, expectations, and individual differences.
- Be gentle and calm with patients and families.
Administrative and Other Related Duties:
- Participate in Performance Improvement activities.
- Participate in Accordia Health committees as required.
- Complete assigned tasks in a timely manner.
- Access community resources as needed.
- Follow Accordia Health Policy and Procedures.
- Maintain current State license and requirements for renewals.
Current LPN or RN license in Alabama. Must possess Basic Life Support (BLS) certification. Knowledge in adult/geriatric and/or child/adolescent specialty, outlined below, to be attained within (6) six months of employment:
- Knowledge of general and psychiatric nursing care.
- Basic knowledge of psychiatric disorders in the adult and/or children populations.
- Demonstration of safe medication administration practices, including IM.
- Demonstrate knowledge of the principles of growth and development over the life span and the skills necessary to provide age-appropriate care to patients served.
- Knowledge of medical terminology, especially those terms associated with adult psychiatric care and/or substance abuse.
- Maintain current knowledge of medications and their correct administration based on patient age and clinical condition.
- Knowledge of behavior management, crisis intervention, and interventions common to acute psychotic and non-violent crisis intervention practice.
- Basic knowledge of appropriate educational methods.
- Understanding of behavioral, psychological, physical health, and social effects of psychoactive drugs, including alcohol and tobacco on consumers and their significant others.
- Knowledge of trauma-informed care principles.
- Knowledge of cultural diversity.
