Job Summary

We're seeking a compassionate and skilled Registered Nurse to join our team as a Home Health Registered Nurse. As a key member of our care team, you will be responsible for delivering high-quality patient care, collaborating with our interdisciplinary team, and promoting a positive patient experience.

Responsibilities
  • Deliver high-quality patient care, including assessments, care planning, and implementation of care plans
  • Collaborate with our interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive patient care
  • Conduct patient and family education, promoting health literacy and self-management skills
  • Document patient information accurately and timely, maintaining confidentiality and adhering to HIPAA guidelines
  • Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices
Requirements
  • RN license in the state of Illinois
  • Graduate from an approved school of nursing
  • One year of RN experience, with clinical competence in delivering quality patient care
  • Current driver's license and ability to spend approximately 20% of work time driving to/from patient locations
  • Commitment to consistently meet critical deadlines for charting
  • Ability to self-manage time and schedule
  • Demonstrated experience with tablets, mobile phones, and EMR software
