Geriatric Palliative Care Nurse Practitioner

Mount Sinai Health System is seeking a highly skilled and compassionate Geriatric Palliative Care Nurse Practitioner to join our team. As a key member of our interdisciplinary team, you will provide high-quality patient care, education, and support to patients and families in our geriatric and palliative care settings.
Responsibilities:- Assess patients' health status through comprehensive health history and physical examination.
- Develop and implement individualized care plans, including therapeutic and diagnostic procedures, consultations, and laboratory examinations.
- Prescribe medication regimens and interpret findings to alter care plans according to accepted standards of care and practice guidelines.
- Provide education and counseling on health promotion, maintenance, disease prevention, and management of acute and chronic illnesses.
- Document findings and care plans in patients' medical records.
- Consult with physicians according to established standards, policies, procedures, and protocols.
- Participate in team meetings and conferences to enhance interdisciplinary care delivery.
- Maintain patient and employee confidentiality in information management.
- Master's degree in nursing or related field.
- Current New York State license as a registered nurse.
- State certification as a Nurse Practitioner with current registration.
- Collaborative Practice Agreement or Collaborative Practice Relationship Attestation.
- Basic Life Support (BLS) certification.
- Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), Neonatal Advanced Life Support (NALS), and The Neonatal Resuscitation Program (NRP) certificates as required by specialty.
