Job Summary

We are seeking an Empathic Support Specialist to join our team at Commonpoint Queens.

This is a 24 hour work week position with a salary range of 20K-30K annually.

Responsibilities
  • Peer Support Services

Our Empathic Support Specialist will provide peer support services to clients of the CAPE Clinic and MIST Program, including orientation to programs, service providers, and community resources.

Safety Assessment and Resource Connection

The ideal candidate will assess clients' safety in the community and home, working with the MIST partnership to connect clients with appropriate resources to maintain safety and independence.

Technology Assistance

They will assist clients in connecting with technology to better engage with mental health appointments, virtual social activities, and connections with family and friends.

Outreach and Engagement

Our Empathic Support Specialist will conduct outreach to members missing appointments to reinforce engagement and assist members in carrying out tasks defined in their treatment plans.

Support and Advocacy

They will attend and participate in various appointments/meetings to support and advocate for clients as required, understanding and addressing related cultural attitudes and practices.

Progress Reporting

The successful candidate will report on clients' progress relative to experience in community settings to clinical team members, referring appropriate questions and issues to clinical and non-clinical team members.

Travel and Documentation

They will travel to member homes to provide peer-level support and maintain appropriate documentation in electronic health records.

Requirements
  • Education and Certification

A High School diploma or equivalent is required; an Associate's or Bachelor's degree is preferred. State certification as a Peer Specialist or Peer Advocate is required or must be obtained within 6 months of hire date.

Communication and Transportation Skills

The ideal candidate should demonstrate support skills and enthusiasm for assisting underserved populations, with reliable access to transportation and telephone, and a willingness to travel.
