BAYHEALTH INC is seeking a highly skilled Home Health Physical Therapist Assistant to join our team. This exciting opportunity will allow you to make a meaningful difference in the lives of patients by providing essential home health services under the guidance of a Physical Therapist.

About the Role:

As a Home Health Physical Therapist Assistant, you will be responsible for carrying out physical therapy treatments as directed by the supervising Physical Therapist, aligned with patient goals and physician orders. Your duties will include providing hands-on care, including therapeutic exercises, gait and functional training, and muscle reeducation. You will also utilize physical agents (heat, cold, ultrasound, etc.) to support treatment goals.

Responsibilities:

  • Carry out physical therapy treatments as directed by the supervising Physical Therapist, aligned with patient goals and physician orders.
  • Provide hands-on care, including therapeutic exercises, gait and functional training, and muscle reeducation.
  • Utilize physical agents (heat, cold, ultrasound, etc.) to support treatment goals.
  • Observe, report, and document the patient's response to treatment, coordinating closely with the supervising Physical Therapist, Clinical Team Manager, and other team members.
  • Instruct patients and their families in home exercise programs and the use of assistive devices.
  • Maintain timely and accurate clinical records, contributing to patient progress notes, care documentation, and interdisciplinary conferences.
  • Adhere to agency standards for using telecommunication and point-of-care technology.
  • Participate in interdisciplinary team meetings and in-service education programs to continually enhance home health care standards.
  • Engage in quality improvement initiatives and support agency protocol development as needed.

Requirements:

  • Graduate from a Physical Therapy Assistant program approved by the Council on Medical Education of the American Medical Association in collaboration with the American Physical Therapy Association.
  • Current California Physical Therapy Assistant license.
  • Minimum of 2 years' experience as a Physical Therapist Assistant, with a preference for experience in gerontology and home care.
  • Valid California driver's license and automobile insurance.
  • Current CPR certification.
  • Demonstrated ability and willingness to use electronic documentation and telecommunication devices effectively.
