Job Summary

We are seeking a skilled and compassionate PRN Registered Nurse to join our Alachua County team. As a PRN RN, you will provide high-quality nursing care to clients in their homes, working closely with our interdisciplinary team to ensure seamless care coordination and exceptional patient outcomes.

Responsibilities
  • Conduct comprehensive health and psychosocial evaluations to identify client needs and develop personalized care plans.
  • Monitor client conditions, provide health promotion and prevention services, and coordinate care with other healthcare professionals.
  • Deliver direct nursing care, including medication management, wound care, and other interventions as needed.
  • Collaborate with clients, families, and caregivers to educate and empower them to manage their health and well-being.
  • Document client progress, outcomes, and care plans accurately and thoroughly.
Requirements
  • Current RN licensure in the State.
  • At least one year of experience in home health care or a facility setting.
  • Knowledge of Medicare and Medicaid guidelines, as well as home health care principles and techniques.
  • Excellent organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals.
