Pediatric Health Coordinator

AmeriHealth Caritas seeks a highly skilled Pediatric Health Coordinator to coordinate services for medically fragile pediatric patients and their families. This key member of our care management program will engage with members and caregivers to establish individual health goals, conduct assessments, create care plans, and navigate health services to ensure optimal patient outcomes through resource management.
About the Role:
The RN Care Management Specialist is responsible for conducting telephonic assessments to determine care coordination and care management needs, creating comprehensive person-centered assessments inclusive of physical health history, mental health history, social determinants of health, and supportive needs, coordinating physical, behavioral health, and social services, providing medication management, identifying problems/barriers for care coordination and appropriate care management interventions, and creating a plan of care to assist members in reducing/resolving problems and or barriers so that members may achieve their optimal level of health.
Responsibilities:
- Conduct thorough telephonic assessments to identify care coordination and care management needs.
- Create comprehensive person-centered assessments that include physical health history, mental health history, social determinants of health, and supportive needs.
- Coordinate physical, behavioral health, and social services to support member needs.
- Provide medication management, including regular medication reconciliation and support of medication adherence.
- Identify problems/barriers for care coordination and implement appropriate care management interventions.
Requirements:
- Bachelor's Degree in Nursing or health-related field.
- ~3+ years of clinical experience at the bedside (as a Registered Nurse) working with medically fragile pediatric patients in an acute care, home care, or special needs clinic.
- ~3+ years of Case Management experience, preferably telephonic within a managed care organization, desired.
- ~Demonstrated proficiency working within a Windows 11 environment to include MS Office (Excel, Word, Outlook) and electronic medical record and documentation programs.
- Valid driver's license and car insurance.
- Ability to work Monday-Friday from 8:30a to 5:00p EST.
