Pediatric Care Coordinator

The Pediatric Care Coordinator will work closely with medically fragile pediatric patients, engaging with members and caregivers to establish individual health goals. This role involves conducting assessments, creating care plans, and navigating health services to ensure optimal patient outcomes through resource management.
Responsibilities- Conduct comprehensive person-centered assessments inclusive of physical health history, mental health history, social determinants of health, and supportive needs.
- Creat a plan of care to assist members in reducing or resolving problems and barriers so that members may achieve their optimal level of health.
- Navigate health services to ensure quality and cost-effective resources.
- Bachelor's Degree in Nursing or healthcare related field.
- ~ 3+ years of clinical experience at the bedside (as a Registered Nurse) working with medically fragile pediatric patients in an acute care, home care, or special needs clinic.
- ~ 3+ years of case management experience, preferably telephonic within a managed care organization, desired.
- Valid driver's license and car insurance.
The estimated salary for this position is $65,000 - $80,000 per year, depending on location and experience.
