The role of the Case Manager is pivotal in assisting clients facing a range of mental health challenges. The primary objective of this position is to support clients in achieving psychiatric stability while fostering the development of essential life skills.
Key ResponsibilitiesWe are currently seeking an Assertive Community Treatment (ACT) Case Manager.
- Engage regularly with fellow ACT Team members to deliver holistic treatment to clients with severe and persistent mental health conditions.
- Conduct home visits to evaluate client progress, provide encouragement, and offer guidance, including weekend rotations.
- Lead individual and group sessions focused on basic living skills as per the rotation schedule.
- Act as an auxiliary case manager when required.
- Continuously update knowledge of community resources and services.
- Effectively relay client needs during team discussions.
- Organize and schedule Basic Living Skills (BLS) group sessions.
- Maintain accurate and timely documentation as per organizational policies.
- Assist clients in arranging appointments within the center and the community.
- Establish and nurture open lines of communication with clients, their support systems, and the team.
- Provide reminders for appointments to clients and their support networks.
- Demonstrate comprehensive understanding of the ACT Program in accordance with DMH and Medicaid regulations.
- Address client crises promptly and follow up as necessary.
- Participate in community outreach activities, including scheduled soup kitchen engagements.
- Facilitate family education and support initiatives as determined by the treatment team.
- Assist clients in accessing safe and affordable housing options.
- Complete additional tasks as assigned by the supervisor.
