About the RoleWe are seeking a skilled Case Manager-Weekends to join our team at WellStone. As a Case Manager-Weekends, you will work directly with clients in our WellStone Emergency Services facility, providing resources for care, assisting the therapeutic team, and encouraging healthy choices as clients transition to outpatient therapy and treatment.
Key Responsibilities- Provide in-patient case management services to clients with mental health needs.
- Assist clients with obtaining medication and monitor medication compliance.
- Provide basic living skills training and transportation to appointments.
- Maintain high-quality client documentation and records management through electronic charting.
- Develop and implement individual service plans, revising goals as needed.
- Assist clients through crises and arrange for assistance from other caregivers.
- Bachelor's degree in a social sciences field.
- Excellent 5-year driving record and reliable transportation.
- Excellent oral and written communication skills.
- State of Alabama Adult Case Management Training.
- Knowledge of community agencies and resources.
- Personal automobile liability insurance in the amounts of $100k, $300k, $50K.
