Key Responsibilities
- Assist in various functions of the admissions department from initial inquiries to discharge and follow-up for our residents.
- Conduct facility tours for prospective residents, whether they arrive spontaneously or through scheduled appointments.
- Collaborate with the Liaison on patient acceptance protocols.
- Monitor daily pre-admission referrals and keep relevant departments informed about anticipated admissions.
- Coordinate and finalize all admission logistics.
- Secure approval numbers from payors on an individual basis as needed.
- Ensure the necessary level of care approvals and conduct required screenings.
- Complete essential documentation and distribute it to the relevant departments.
- Verify eligibility and estimate co-pay/family contributions through the appropriate social service or Medicaid offices when required.
- Oversee acute hospital admissions to ensure patient readmissions.
- Facilitate weekend and holiday admissions in partnership with Liaisons.
- Proactively complete specific elements of the facility admission packet with patients or their representatives.
- Maintain the Resident Roster, waiting list, and inquiry list in compliance with state regulations.
- Coordinate clinical assessments and screenings to ensure timely clinical acceptance in the absence of the Liaison.
- Drive healthcare innovation by introducing new ideas and processes to continuously improve outcomes.
Qualifications
- High school diploma or GED equivalent is required.
- A minimum of one year of experience in Healthcare, Business Development, or Insurance, with proficiency in medical terminology.
- Must be skilled, compassionate, and committed to delivering outstanding results.
