Join a Compassionate Care Team as a Home Health Nurse

At CenterWell Home Health, we prioritize health and well-being. As a Home Health Nurse, your role will encompass:

  • Conducting admissions, managing cases, and performing follow-up nursing visits for patients in their homes.
  • Providing ongoing care and case management tailored to each patient, ensuring follow-up as per the Clinical Manager's directives.
  • Collaborating with physicians to establish initial treatment plans based on medical orders and patient assessments.
  • Delivering hands-on care, evaluating care plans, and educating patients in line with physician directives, under the supervision of the Clinical Manager.
  • Adjusting care plans in consultation with physicians based on continuous assessments and regulatory requirements.
  • Coordinating care among various healthcare professionals, including Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
  • Continuously updating the patient's physician and Clinical Manager regarding patient care, condition, and progress.
  • Implementing care plans collaboratively with patients and their families to achieve optimal health outcomes.
  • Overseeing the work of Home Health Aides and Personal Care Workers, providing clear personal care instructions that align with the current care plan.
  • Monitoring documentation completion by home health aides as part of supervisory responsibilities.
  • Facilitating patient discharges in consultation with physicians and Clinical Managers, ensuring all clinical documentation is accurately completed.
  • Preparing comprehensive medical documentation for all patients, including case conferences, patient interactions, medication changes, re-certifications, progress updates, and care plan modifications.
  • Summarizing patient records and collaborating with other healthcare professionals to ensure optimal patient care.

Essential Qualifications:

  • Diploma, Associate, or Bachelor’s Degree in Nursing.
  • Preferred minimum of one year of nursing experience.
  • Strong background in medical-surgical, ICU, ER, or acute care settings.
  • Experience in home health is advantageous.
  • Current and unrestricted Registered Nurse licensure.
  • Valid CPR certification.
  • Excellent organizational and communication skills.
  • Possession of a valid driver's license, auto insurance, and reliable transportation.

Scheduled Weekly Hours: 24

Compensation: The salary range reflects a good faith estimate for full-time employment, varying based on geographic location and individual qualifications, with an annual range of $69,800 - $96,200.
