Hospice Nurse Practitioner Position in Palliative Care

Haven Hospice is seeking a skilled and compassionate Nurse Practitioner to join our team in providing high-quality care to patients and families in need. As a key member of our interdisciplinary team, you will play a vital role in delivering comprehensive hospice services and making a meaningful difference in the lives of those we serve.
Responsibilities- Provide medical care and manage terminal illnesses in compliance with state regulations under the supervision of a collaborating physician.
- Evaluate patient medical conditions and health histories through history and physical examinations.
- Complete face-to-face visits and assess eligibility for hospice care.
- Maintain accurate clinical notes and communicate effectively with healthcare teams.
- Participate in interdisciplinary meetings to meet patient and family needs.
- Master's degree in Nursing from an accredited program.
- Current nursing license and DEA license (if required by state regulations).
- CPR certification and accreditation from an approved certifying body.
- Minimum one year NP experience, preferably in hospice or palliative care.
- Certification in a specialist area preferred (e.g., CHPN or Geriatrics).
