About the Job
Haven Hospice is seeking a compassionate CNA/Hospice Aide to provide personalized care and support to patients in need. As a key member of our team, you will play a vital role in ensuring the comfort, dignity, and well-being of our patients and their families.

Job Summary:
We are looking for a dedicated and caring individual to join our Palatka Care Center as a Full-Time Hospice CNA/Aide. In this role, you will work closely with our hospice nurses and therapists to provide top-notch patient care, promoting a positive impact from day one.

Key Responsibilities:
* Perform personal care needs for assigned patients, attending to their requests promptly and respectfully
* Report any concerns or issues to the Case Manager immediately
* Provide excellent communication techniques, maintaining confidentiality and respecting patient boundaries
* Meet safety needs of patients, using equipment safely and properly (e.g., foot stools, side rails, O2)
* Assist with daily living activities, such as bathing, dressing, grooming, and feeding
* Take and record vital signs, including temperatures, pulse, respiration, and blood pressure, when ordered and competently performed
* Monitor patient skin integrity, preventing breakdown and reporting any concerns to the Case Manager
* Collaborate with caregivers and other individuals living with the patient, addressing interpersonal issues and providing emotional support
* Assist with ambulation, exercise, and transfers, as directed by the hospice nurse or therapist

Requirements:
* Meet state training requirements for CNAs/Hospice Aides
* Completed at least 75 hours of classroom and supervised practical training, with 16 hours of classroom training prior to 16 hours of practical training
* Minimum six months of experience providing personal care, with preference for those with hospice-specific experience
* Ability to read and follow written instructions, document care given, and maintain confidentiality
* Strong oral and written communication skills, with tact, patience, and good personal hygiene
* Possess current CPR certification and demonstrate ability to work with minimal direct supervision
