About the RoleThis is a part-time position with a competitive hourly rate of $36.00-$55.00. You will be responsible for providing rehabilitation and coordinating medical needs for clients, as well as monitoring and administering medications. You will also be required to attend weekly team meetings and participate in ongoing education and training.
Responsibilities- Provide rehabilitation and coordinate medical needs for clients
- Monitor and administer medications
- Attend weekly team meetings
- Participate in ongoing education and training
- Conduct assessments and approve plan of care and service delivery methods
- Perform competency and skills evaluations on employees who provide direct care
- Inform the physician and staff of changes in the client's medical or mental condition and needs
- Write progress notes, summaries, and discharges
- Implement the treatment/rehabilitative activities and strategies
- Report significant/major incidents or accidents and notify proper authorities of circumstances or situations that present possible health hazards
- Follow consumer health plans and provide and promote consumer choice and facilitate independence
- Assist in maintaining and updating nursing, pharmacy, infection control, and health and safety policies and procedures
- Graduate of an accredited school of Nursing
- Current Registered Nurse licensure in State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hire
- Bachelor's degree preferred, with one (1) year of home health experience preferred
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist
