Job Title: Home Care Coordinator

Nascentia Health is seeking a skilled Home Care Coordinator to join our team. As a Home Care Coordinator, you will play a critical role in developing and managing home care plans for patients referred from hospitals, rehabilitation centers, or skilled nursing facilities.

Key Responsibilities:
  • Conduct assessments and screenings to determine the appropriateness of home care services
  • Manage referrals and visit facilities daily
  • Complete patient visits in the field and ensure the continuity of care for all assigned clients
Performance Responsibilities and Standards:
  • Promote continuity of care for referred clients
  • Determine client/family home care needs upon referral for home care
  • Assist in the development of a home care plan, including the arrangement of needed services, supplies, and appliances
  • Review home care policies and procedures with clients/families, physicians, agency care providers, insurance providers, and service and equipment vendors as indicated
  • Act as a liaison between Nascentia Health and hospitals, rehabs, and/or SNF
Requirements:
  • Current license to practice as a Registered Professional Nurse in New York State or degree in a health-related field
  • Minimum one (1) year home health care experience preferred
  • Current New York State driver's license and automobile insurance required
  • Excellent verbal communication skills required
Physical Requirements:
  • Speaking/visual/hearing ability sufficient to comprehend and express written/verbal communication
  • Ability to perform tasks involving physical activity, including pushing/pulling, lifting/carrying, and patient transfers
