Job Summary

The Wellness Liaison will serve as a key ambassador for Sun River Health Valentine Lane, promoting and educating referral sources, clients, and community members about all SRH services while providing exceptional customer service.

Key Responsibilities
  • Participate in outreach and educational events to promote SRH services and engage with the community.
  • Serve as a liaison between clients, providers, and account management staff to ensure seamless communication and coordination.
  • Provide outbound reminder calls to clients prior to their scheduled appointments to ensure timely attendance.
  • Assist clients in securing appointments for SRH Health Center services and conduct Health Home pre-eligibility screenings and intakes for eligible clients.
  • Achieve monthly intake targets set by management and adjusted quarterly as needed.
  • Support client outreach and education of SRH services to the community, with a focus on linkage to Health Center and Health Home.
  • Assist in arranging transportation when needed and support clients with health insurance applications.
  • Enter complete, accurate, and timely data into required databases, including demographic information, insurance information, assessments, and progress notes.
  • Provide necessary documentation and information to the Departmental Health Home Manager and participate in community events, such as health fairs, to promote SRH services.
  • Act as an initial service ambassador to promote and educate referral sources, clients, and community members about all SRH services while providing exceptional customer service.
  • Support SRH's mission, values, strategic goals, and high standards of customer service, adhering to the Corporate Compliance Program and confidentiality of HIPAA-protected health information.
  • Participate in team meetings and support the overall goals and objectives of Sun River Health Valentine Lane.
