About the Role:- Collect and record patient care data, including initial evaluations, daily notes, reassessments, and discharge plans.
- Develop and implement treatment plans based on patient evaluations and goals.
- Collaborate with interdisciplinary teams, patients, and patient support systems to ensure effective care.
- Provide education and training to patients and their support systems on therapeutic activities, rehabilitation equipment, and community reintegration.
- Oversee support staff, including physical therapist assistants, athletic trainers, and physical therapy aides.
- Bachelor's or Master's Degree in Physical Therapy required; Doctorate degree preferred.
- Licensed Physical Therapist in the state practicing in.
- Completed state-regulated CEU requirements in the appropriate state (NY, NJ).
- Valid CPR license.
- Experience/knowledge with EMR software preferred.
- Excellent oral and written communication skills.
- Strong clinical decision-making skills.
