Job Title: RN - Respite

Phaxis is seeking a skilled Registered Nurse to join our team as a Respite Care Specialist. In this role, you will provide high-quality care to individuals with chronic mental illness and substance abuse issues.

Responsibilities:
  • Conduct psychiatric assessments and physical health needs assessments
  • Make appropriate referrals to community physicians and provide management and administration of medication
  • Provide a range of treatment, rehabilitation, and supportive services
  • Ensure and maintain principles and procedures in agreement with Nursing licensure, Program guidelines, and Department of Mental Health and Office of Mental Health regulations
  • Develop, maintain, and revise written plans of care and general health care records
  • Engage new clients in the continuum of care through outreach
  • Collaborate with other health care providers and make appropriate referrals
  • Serve as client advocate specifically in accessing the health care system
  • Provide communication to team members and participate in community and apartment visits to clients
  • Coordinate services with other community mental health services
Requirements:
  • Bachelor's Degree in Nursing
  • Experience with chronic mental illness and substance abuse individuals
  • Preference given to persons with experience on a multidisciplinary mobile team
  • Must have hospital discharge experience
