Job Summary

We are looking for an experienced RN to provide high-quality patient care and support to individuals and their families in need of hospice services. As a Hospice RN, you will work closely with our interdisciplinary team to develop and implement personalized care plans, ensuring that each patient receives the highest level of comfort, dignity, and compassion.

Responsibilities

  • Provide direct patient care services, including assessment, planning, implementation, and evaluation of care plans;">
  • Act as primary liaison between physicians, patients, and families, ensuring seamless communication and collaboration;">
  • Educate patients and caregivers on various aspects of care, empowering them to make informed decisions;">
  • Manage and coordinate care with other healthcare professionals, including LPNs, CNAs, and support staff;">
  • Maintain accurate and detailed clinical records, adhering to industry standards and regulations;">
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.">

Requirements

To be successful in this role, you will require:

  • ">
  • A valid RN license in the state of employment (or eligibility to obtain);">
  • At least one year of nursing experience in hospice, home health, ICU, geriatric, or oncology; and">
  • Demonstrated flexibility, composure, and empathy when working with patients and families in crisis situations.">
