**Job Description:**

We are seeking a skilled Registered Nurse to join our team as a Nursing Professional for Palliative Care. As a member of our interdisciplinary team, you will be responsible for coordinating and providing high-quality patient care to individuals and families in need of palliative care services.

**Responsibilities:**

  • Conduct thorough assessments of patients and families to identify their unique needs and develop individualized care plans.
  • Collaborate with healthcare providers to ensure continuity of care and optimal patient outcomes.
  • Provide education and support to patients and families regarding end-of-life care and symptom management.

**Requirements:**

  • Current licensure as a Registered Nurse in the state.
  • Minimum 2 years of experience in home care, public health, or hospice nursing.
  • Excellent communication and interpersonal skills.

**Salary Range:** $90,000 - $120,000 per year, depending on experience.
