We are seeking a skilled Licensed Vocational Nurse to join our team at Renewal Health Group. As a LVN, you will play a vital role in providing community substance use nursing services, focusing on prevention, treatment, education, and rehabilitation of physical and mental health.
Key Responsibilities- Medication Management: Assist with self-administration of medication policy, document and observe patient medication regimen, and ensure accurate documentation of medication orders.
- Patient Observation: Observe patients, charting and reporting changes in patients' conditions, such as adverse reactions to medication or treatment, and take necessary action.
- Healthcare Team Collaboration: Work as part of a healthcare team to assess patient needs, plan and modify care, and implement interventions.
- Medical Support: Evaluate medical concerns, direct patients to emergency medical care when required, and provide monitoring and supervision of patients to support non-clinical staff.
- Administrative Tasks: Request inventory, requisition supplies and instruments, schedule and transport clients to medical and dental appointments, and facilitate Health and Wellness groups.
- Attainment of at least 18 years of age.
- LPT licensure required.
- Experience working in a substance use facility preferred.
- Experience working with clients in Acute withdrawal from addictive substances, including COWs and CIWA assessments.
- Demonstrated ability to work as a team player, excellent communication skills, and a valid driver's license with a clean driving record and personal automobile insurance.
- Hiring is contingent upon completion of a Background Check, TB test, and clearance on a drug screen and pre-employment physical.
- Current First Aid and CPR certification or willingness to obtain upon hire.
- DOJ/FBI fingerprint clearance or an exemption and a submission of a signed Criminal Record Statement (LIC 508).
- Vaccination requirements as per company policy.
