Responsibilities:
- Assess patients and develop individualized treatment plans to meet their physical and psychosocial needs.
- Consult with physicians and nursing staff to ensure comprehensive care.
- Initiate requests for consultation or referral and respond to requests from patients, physicians, or nursing staff.
- Examine patients and their records to identify early stages of serious physical, emotional, or mental problems.
- Perform administrative duties, including completing medical forms, reports, evaluations, and charting.
Qualifications:
- Current, unencumbered, active license to practice as an LVN in the state of CA.
- Current, active CPR certification.
- Knowledge of nursing and medical practices, as well as laws, regulations, and guidelines that pertain to healthcare facilities.
- Strong interpersonal and communication skills.
- Ability to read, write, speak, and understand the English language.
